Pre-Admit Instructions
Pre-Admit registration is required and must be completed at least 2-3 days prior to your surgery. The purpose of this is to ensure that the facility is aware of the patient's medical history and to ensure that the patient is prepared for the upcoming procedure. An appointment is not required and can be done Monday-Thursday, 6:30am - 5:00pm & Friday, 6:30am - 2:00pm. You will meet with a patient coordinator to sign consents and discuss insurance benefits. You will be notified of any required deposits at that time. Pre-Admit may include completing additional tests (lab work, EKG, etc.) if ordered by your surgeon.
For Pre-Admit
We Request that you bring:
- The packet provided by your physician's office (includes orders & consents)
- Medications
- Insurance card (not needed if through worker's compensation)
- Driver's license or state issued ID
